POLICIES AND PROCEDURES Cancellation and Missed Appointment Policy: Scheduled appointment times are reserved especially for you. If an appointment is missed or it is canceled with less than 24 hours notice from the beginning of your scheduled appointment, you will be billed. We will make an attempt to contact you to confirm your appointment on the day before your appointment. Please keep us current on the best contact phone numbers for you. This call is a courtesy and our failure to reach you will not relieve you of your responsibility for any missed appointment charges. Cancellations must be made during office hours (M.,Tu., & Th. 10am-5:30 pm, Wed.at Dr. Berger's discretion from 1-4pm, & Fri.closed). Failure to keep scheduled appointments without notification will be charged and can lead to dismissal from the practice. Please help us to serve you better by keeping your regularly scheduled appointment. Termination of care: Termination of care may be due to the consumer’s own desire to leave treatment. If you have not had a follow-up appointment in more than 6 months, your chart will be closed and I will consider our relationship terminated. Non-compliance with medications and/or non-compliance with treatment plan shall be grounds for termination. Smoking Policy: There is no smoking in the entire building. Prohibition of illegal/Illicit Drugs or Alcohol: The possession, consumption, or distribution of illegal/illicit drugs or alcohol within my office is strictly prohibited. It is my intent to provide a safe environment that contributes to the treatment process. Therefore, guns, knives, and any other objects used to intimidate or cause bodily harm are prohibited. |
