FINANCIAL TERMS AND FEES: My fee for service is: Initial Psychiatric evaluation (1st visit) .................................. 90" / $525.00 No insurance is accepted at this time. All charges and fees are due at the time of service in the form of check, cash, or credit card unless previous arrangements are made. Past due charges and fees must be paid in full (or a payment plan is pre-arranged) before follow-up appointments can be scheduled. OTHER FEES FOR ADDITIONAL SERVICES: Paperwork charges: * Prescription call-ins because a follow-up appointment was not scheduled at the previous appointment or due to a broken or a rescheduled appointment will be billed $35/ rx. * Fees for lost prescriptions can be discussed at the follow-up appointment. * Phone calls: (please only urgent calls after hours) * Simple calls (<5 min) related to clarification on medication side effects or instructions on taking medications: No Charge * More complex calls, calls in lieu of face to face appointments as a result of scheduling conflicts will be charged at a rate of $450.00/ hr (fee to be prorated in 5 minute blocks of time). * Missed medication management appointments will be charged full rate * Contacting insurance company for Prior Authorization for med coverage: $50.00 * Copying records: $45 |
